It's when the couple has The Conversation and both parties agree not to see anyone else.

I think a conversation is warrented, but when I date someone, if there is a second date coming, I consider it exclusive. I only date one person at a time out of respect for the person.

Usually becomes exclusive when the engagement ring goes on (or should anyway).
