It is a fairly simple test to determine if a person has PCOS or not. It involves a blood test. My daughter was diagnosed with PCOS earlier this year through an unusual series of events. She went to the doctor for an ingrown toe nail. The doctor who removed the toe nail noticed several unusual moles and suggested they be removed. The doctor who removed the moles was looking aroung her neck, shoulders, and back for other moles and noticed the hair growth on her chest and back. She asked if my daughter had ever been tested for PCOS and my daughter said she had never heard of it (nor had I). That doctor sent her for the blood work. In addition to difficulty losing weight, carrying her weight mostly in her stomach, male-pattern baldness, irregular periods, and unusual hair growth, her testesterone level was high as was her blood glucose level. She went for an ultrasound of her ovaries and has cysts on both of them. It has been a long few months for her trying to adjust to taking birth control pills to regulate her periods and take Metformin for the Insulin Resistance. She has had some issues with side effects. Your sister will likely find losing weight a difficult endeavor without some medical help. Women with PCOS often have a very hard time losing weight even when actively trying to. Our family practice doctor was the one who diagnosed PCOS for my daughter, who will be 23 in Novemeber. As the mother of a daughter with PCOS who has done some research, I suggest that your sister first go to the doctor (it doesn't matter if it is a general practioner or an ob/gyn). If she has been trying for 6 months to get pregnant, then the ob/gyn may be the best way to go. I say forget about losing weight first (as it won't likely happen quickly or easily for her). Not all doctors are familiar with PCOS so she may have to try a few doctors to find one that is knowledgable. Our family doctor happened to have a sister with it and recognized the potential signs, but she said most women find out they have PCOS when they are trying to conceive. There are numerous medications that can be prescribed for your sister if she has PCOS and she needs to find the right combo for her which may take a while.

Weight loss will not affect a patients diagnosos, which will be done by an umtrasound scan.
Weight loss can be more difficult for a PCOS sufferer, but it really doesn't matter whether you start now or later, but you may feel better if you start sooner. Weight loss will make TCC easier.
Good luck, hope all is well.

Have her thyroid checked!! Have her doctor test her TSH [thyroid stimulating hormone] and T4Free. It sounds like she is hypothyroid!
[If she is hypothyroid .. that can cause weight gain, and irregular periods, and unable to have children until the thyroid levels are normal with medication]
I have had hypothyroid since I was 12 [it was diagnosed before I became overweight, I got checked because both of my parents have thyroid conditions. My dad is hypo and my mom is hyper .. so low and behold, I got it too!]
It's easily fixed. You just take 1 pill a day [Synthroid] and it will stimulate your thyroid to function better. Once her levels are normal, she will be able to have a perfectly healthy baby like anyone else :]
good luck! God bless!

She needs to start working on her weight this very second. I wouldn't even THINK of TTC while being so grossly overweight. It's bad for the baby.
She may not even have PCOS so I suggest she sticks with losing weight and of course she needs to speak with her obgyn. She should get on prenatals while TTC as well.
