They hand you a gown and tell you which area to disrobe. i.e. "Here's a gown for you. Please disrobe from the waist-up (or waist-down, or please disrobe completely) and the doctor will be in to see you in a moment. They always leave the room to give you time and then knock upon their return.

There are valid reasons why you need to have your clothing removed so the doctor can exam you. You are given a gown and the doctor is aware that you may be uncomfortable but often so is the doctor. It is a professional exam and done with care to make you as comfortable as possible. There's nothing personal about it and no reason to be embarrassed. It's just a part of health care. There's no need to say anything.

That's an interesting question.. I guess it depends if it's justifiable. Like if I go to the doctor and say that I have a problem with my back then sure I would just take of my shirt so they could see my back.
You should use common sense to judge if something is amiss. If you don't trust the doctor just ask them why.

The nurse that usually walks you in the room will tell you in advance to disrobe completely or just the waste down and they would personally hand you a gown to get in to if needed. If they don't tell you anything first then it could be that the doctor would like to get a history first before any physical examination.

Usually a nurse will say "Here is a robe for you to put on. The doctor will be with you soon" and leave you alone for a few mintues to get changed.
Then the doctor knocks before coming back into the room to ask if you are ready.

UHUH! all answers are mostly the same. but i would like to tell you, when i was in another country and got so sicked there, i went to the hospital and was checked by a "man" doctor. yeah, he told me to take off my clothes and take xray of my lung. we were only two at the time since the nurse went out to get an injection. after changing the hospital robe, you want to know what the doctor told me? he said: "foreigners are really great. nice body and good looking. so perhaps they're good in bed also"!!!. when the nurse came back, i told her what the doctor told me. but the doctor said, oh sorry, but i only admire foreigners. and so i told the doctor also: "you're such a nice and ***** doctor of your f4cking country, thanks for the xray".
