I've been barefoot at my job in a history theme park; we dress up in period clothing and in Prehistory, bare feet fit quite well (although we have moccasin-like footwear, too). Before that I've also worked at an internet help desk barefoot, and I've done a couple very brief temp jobs in winter barefoot, like packing rubber products in a factory (the theme park is closed in winter so sometimes I get a temp job if I can't carry over until spring).

No, always business/casual shoes. But, sometimes we take off our business/dress shoes (socks/stockings/pantyhose) only at our desks/cubicles. :D
