Do you mean dissertation? That is a paper written and presented by a student for a Ph.D. or perhaps a Master's degree. If you want to access someone's dissertation check with the library at that school. They usually have a copy on file.

**additional info
Other authors probably go to those university libraries and read the dissertations cited in their articles. Maybe someday in the future all such dissertations and theses will be digitized but we are not there yet. They may also write (or email) the authors of the dissertations that interest them and ask for a copy. Sometimes a dissertation may be published in a scientific journal or the proceeds of a symposium or other meeting.

If you are looking for a dissertation (Ph.D.), unless that school has digitized its archives, you may have trouble being able to download it. Many just put hard copies in their libraries. Masters' theses are even harder to get, because generally the school itself does not keep them, but if they are kept at all, they are kept in the office of the particular program from which that person got a degree. In other words, not everything is available online for downloading.
