the words "how do you do" are words of etiquette. When I respond to that I get eyebrows raised as most people don't know how to respond to that question. The eyebrows raised are because the person is surprised that people still come from good breeding.
The answer to that phrase "how do you do" is "very well thank you" and how are you today? then you talk about the weather.
If you are introduced to someone you can either say "how do you do" or "pleased to meet you".
What do I say when meeting someone for the first time? Since I'm in business I shake their hand and say "how do you do". This is also used for social occasions.
When people are in jeans and they introduce me to someone I would say "hello, how are you" or "hi, I didn't catch your name" or "hi, are you from xxxx (this city, or are you visiting)".

say that a few times in a row, tongue twister. that phrase is from so long ago, don't hear it at all nowadays. just the normal greeting, hello, nice to meet you and then shake hands. if the person is a bit away from me, then I nod.

I'd say "How's it going?" or something. It's not too engaging so it leaves an opportunity for me to either build on it or leave before I get in a conversation I honestly can't be bothered with, ahah.

I usually stand up, offer a handshake, and say "Hi, how are ya?"
I also find that sticking the person's name in there "Hi Joe, how are ya?" helps me to remember them later.
On a side note, men that do not stand when they offer a handshake irritate me.
