Batters may switch sides during their at bat, but the batter must remain on his established side of the plate when the pitcher is ready to pitch and time is in. The batter may ask the umpire for a time out to switch sides.

Rule 6.06 (b) states the batter is out if "He steps from one batter's box to the other while the pitcher is position ready to pitch:"

No mention in the rule book about two strikes. So the batter can change at any time of the count, as long as the pitcher is not in the set position.

A player can switch during an at bat AS LONG AS there are not 2 strikes.

Yes, as long as time has been called and the pitcher is not in his set position.

Yes, as long as the pitcher is not in position and ready to pitch. (Which is not the same as the pitcher being in the "set" position.)
