Different schools have different requirements. Some do require the GRE, others will want the MAT or GMAT. Some require the GRE subject, others don't. It's very variable and also depends on the major.

Most will require some exam just to determine what your present level is. If you're serious about getting into a good PhD program then take the GRE, MAT, and GMAT and see how you do. Then you can pick the best school for you based on your exam results.

The real answer to your question is "no, you don't always need a GRE to do a PhD in the US" but, you usually need it and the best schools will require it. You may also need a TOEFL score.

Not all schools require the GRE. I am getting my PhD from Walden University which is an online school and I did not need the GRE. My best advice is ask the school that you want to apply to if they require it.

Yes, a GRE or its equivalent (GMAT for business) are always required for any reputable institution's doctoral program. The ones that do not require are an instant sign of being a degree mill -- they are only interested in your credit card # and have no other barriers.
