"Bloody Mary" actually refers to Mary I of England (Mary Tudor born on18 February 1516 – 17 November 1558) was Queen of England until her death. She was remembered for briefly returning England to Roman Catholicism and had almost three hundred religious dissenters executed by burning at the stake, as a consequence, she is often known as "Bloody Mary." She died on November 17, 1558 at the age of 42, most likely of ovarian cancer.
Bloody Mary, is a ghost or witch said to appear in a mirror when her name is called three times. One of the more common ways participants attempt to make her appear is to stand before a mirror in the dark (most commonly in a bathroom) and repeat her name three times, though there are many variations. Some include chanting a hundred times, chanting at midnight, spinning around, rubbing one's eyes, running the water, or chanting her name thirteen times with a lit candle.
In some versions of the legend, the summoner must say, "Bloody Mary, I killed your son!" or "I killed your baby." In these variants, Bloody Mary is often believed to be the spirit of a mother (often a widow) who murdered her children, or a young mother whose baby was stolen from her, which made her go mad in grief and she eventually committed suicide. In stories where Mary is supposed to have been wrongly accused of killing her children, the querent might say "I believe in Mary Worth." This is similar to another game involving the summoning of the Bell Witch in a mirror at midnight. The game is often a test of courage, as it is said that if Bloody Mary is summoned, she would proceed to kill the summoner in an extremely violent way, such as ripping his or her face off, scratching his or her eyes out, driving the person insane or bringing the person into the mirror with her. Other variations say that the querent must not look directly at her, but at her image in the mirror; she will then reveal the querent's future, particularly concerning marriage and children.
As for the legend of "Bloody Mary" is only a myth. Sometimes your brain likes to trick your eyes, it could be your own shadow or image through the mirror. As the legend goes, you have to twirl around 3 or 13 times, this is to make your head dizzy so you could only see blurry images in front of you. Since you can't really see what is in front of you, you start to make up your own interpretation of what you saw.
Injuries and accidents could occur when people try to scary themselves. Especailly in the dark, they could get really scare, run into things such as breaking their noses (cause of bleeding) or banging their heads into a wall (cause of bruises).

The legend goes that Mary was murdered by her lover in front of a mirror and now when you stand infront of the mirror at night and say "bloody mary" three times she comes and scratches your eyes out.

Ive tried it, doesn't work, we also have the candyman legend here in australia, which is sort of the same, you stand infront of the mirror and say "candyman" 3 times, and legend goes he was a serial killer and hee comes and rips your heart out or something, but alas nothing happens.

What you do is light candles , then turn all of the lights off
And if you want it to work you haft to turn the tap
On and chant Bloody Mary 3 times and she will come.
