If you don't have any "physical evidence" such as expelling the fetus in the toilet... the only things that may give an indication might be painful abdominal cramps, a bloody show that is much more than just spotting.
The general rule of thumb is that if you feel in your gut that something is wrong... trust you instinct and go to the doctor.
It's better to be safe than sorry.

If you had a positive pregnancy test, confirmed by a doctor and ultrasound, then a follow up ultrasound to determine if the pregnancy has continued.
Miscarriage has many symptoms. painful cramping and heavy bleeding are majority.

I believe that if you have a blood clot. That is a good sign of a miscarriage.

A doctor only can answer this, because some symptoms as bleeding for example can mislead you.

It all depends on many weeks you think you're pregnant.
If you're still in early stage,go to your doctor, have your urine test for pregnancy. The doctor will do vaginal examination and an ultrasound is necessary, too.
