If you have a tight foreskin, you can actually stretch it like I did when you shower. =)
Work on getting it less tight. Here's what you do. When you shower and soaping up your body, pull the foreskin back the most you can without forcing it, and then wash the head there underneath. If you do this for a few weeks each day, you'll see results. It'll slide back a little more every few days.
(and this part is in case your head is too sensitive to touch; might not be if you've been able to pull the foreskin back when soft) Now, as for the sensitivity, as studies have proven, the foreskin keeps the head sensitive. That's not a bad thing, since that creates pleasure. However, you don't want it too sensitive. Yours is just too sensitive because it hasn't been used much before. So you just have to rub it gently at first for some weeks and you'll see the difference. Then you'll be able to rub the head however you want. =) So in a way, those two things are tied.

You do NOT need circumcision. Circumcision does carry risks and most 'benefits' are unfounded or contradicted. There's a reason why the USA is the last developed nation doing it to newborns, and our rates are falling; it's as low as 14% in some states by now compared to being 90% back in the 70s and 60s. Studies have even proved that circumcision reduces sensitivity. There's other risks (such as more difficulty with masturbation, etc, in the following link)

If you work on it, everything will eventually be fine, as it was for me. (it hasn't given me any problems ever since I stretched it)
In the end, if the stretching fails after a few weeks, you can ask a doctor for a cream. It's 0.05% betamethasone, which is successful in over 90% of cases. You can even take the study to him. It's a really inexpensive cream; Wal-Mart sells it for $4 with prescription and online you can buy it for about $10 at drugstore.com, for example.

The doctor will not care about your penis, he's seen hundreds before yours, and won't be bothered if you get an erection or whatever out of embarassment.
He will definitely be able to help you reduce the pain and help loosen your foreskin. The last resort, if all else fails, would be circumcision. But only consider this if your tight foreskin appears incureable, as it is an irreversible procedure that may not be a necessity.

Soak in a warm tub to loosen the skin and try pulling it back. It won't go all the way at first, but after several weeks of doing this you should be able to. If not, see your doctor.

i think it is pretty crappy for a "nurse" to tell you to "grow up" and visit a doctor.
It is completely understandable to be imberrassed about asking your doctor about something like that.... just remember that if it can be seen, the doctor has seen it. He wont tell on you, or point you out.
Just go. I would.

you might just want to stretch it a little everyday... preferably after the shower in the morning... do the stretch up to the point where it is comfortable to you... it is just skin anyway... so it can easily be stretched...
and then there are creams that can help you... but do the stretching first... no cost to it... of course... do it soon... the older you get... the tougher your skin gets... the harder it is to 'train' the skin...
