USN has a de facto requirement of a bachelors. It's nearly impossible to get into that school without a bachelors. Some schools do require a bachelors.

Right now, a bachelors is not required at all pharmacy schools. Some schools do require a bachelors and others have de facto requirement.
General prereqs at most schools are biology, chemistry, physics, organic chemistry, microbiology, anatomy/physioloy, public speaking, statistics, calculus, economics, freshman english composition, and some general education courses.
Most schools also require that you take the PCAT exam.
You can get a masters with your PharmD.

First off, the process for qualifying as a pharmacist differs from jurisdiction to jurisdiction. You should go to the website of the Faculty (or School or whatever) of a university near you or which interests you and check its admission requirements.
For the second question yes you can get graduate degrees in pharmacy.

why don't you find a school that you want to go to that has a pharmacy program and read their site on the program? I live near Rutgers in NJ and they have a pharm major that IS BS degree (it's a 6 year program). other schools might have something similiar.
