I want to change it from my name and gift it to my brother and there is no money being exchanged, also there are no liens on the property.

Your best bet is to go to your local title company and ask them to explain it to you, maybe ask them to do a quick chain of title search and a name search on you and your brother on any potential liens (such as taxes). The title company will prepare the conveyance document that you choose after the explanation. If they charge you it will be minimal.
Then it will be signed, notarized and you will have to give them the recording fee so that they can take it to the courthouse and record it for you. Their underwriters are attorneys.
Another concern I would have for you, is before you accomplish the conveyance you should contact whoever prepares your taxes and ask them how this transfer will affect you and your brother's taxes both state and federal.

You may need help with the gift tax return. Any gift over 12K requires the giver to file a return even if they owe no tax.
Also if you are doing it to avoid paying for a nursing home for yourself it may not hold up. They look back to see if you were trying to defraud Medicaid.

assuming in USA, you may want to talk to a CPA for the tax consequences but in general you will be making a transfer of title simple process the county clerk should have the forms

you still have to have someone prefare and file the new deed..which will be a small cost..so find a attorney or title company...you can also look into a quit claim deed....
