The Fastest Observed is Mark Wohlers with a 103 mph fastball in 1995 spring training, but the guiness book of world record holder for fastest pitch is Nolan Ryan with a 100.9 mph fastball. But, Joel Zumaya has thrown a few pitches over 103 mph, but it is not observed because of radar guns accuracy

mmmm... i know billy koch threw one clocked at 101.
farnsworth has hit 100 once or twice.
mark wholers may have.
i heard once that bob feller could hit 105mph....i'm not too sure about that!
depends on the gunn, a juggs gun adds 2-3 mph to a players speed.
