It was one of many cruel things that victors would do to their victims in ancient times, although torture and death were more common for the masses; crucifixion was one such torture method invented by the ancient Persians. Certainly the Babylonians and Persians would practice it in some cases, especially when they took captive the best of the enemy's young men and desired to bring them into their own service; this was how ancient kingdoms would acquire their eunuchs who then made good servants. It's very likely that such happened, for instance, with the biblical case of young Daniel and the other Hebrews taken captive to be educated in the Babylonian court.

it was common practice for the nobility to castrate people serving for the king in order to prevent the rise of other dynasties. so maybe if a stronger tribe were to enslave captured soldiers and have them working for the nobles, then castration would take place in order to prevent that particular person from starting an uprising and usurping the throne and starting a new dynasty. this was common practice in china, ottoman turkey, persia, rome, etc.

That would really depend on the culture and the time period.
I personally can't recall any but I suppose it's possible. I'm more familiar with executing or enslaving the losing side.
