It's complex...
I use my index finger on the right hand for the left mouse button, and my ring finger on the right hand for the right mouse button. I use the middle finger for the scroll wheel/toggle.
If I'm using a travel mouse, I use the index finger and the middle finger for the left & right mouse buttons, because the mouse is smaller/narrower.
On my laptop, when using the built-in trackpoint mouse, I use the middle finger for the trackpointer and the thumb for the left and right mouse buttons. Never use the middle mouse button on the trackpoint for some reason.

My right index finger, even though I'm left handed, I can't use the mouse with my left hand! It's wierd.
