what kind of opener is it? On chamberlain, craftsman, and liftmaster, it is a problem they have. They have available a new trolley for it. The problem is where your tightening adjustment is, the bolt hole is round. This allows the bolt to turn. They have a new trolley available that instead of a round hole it is more of a D shape. check http://www.chambelain.com and or http://www.liftmaster.com

I will trust you mean a Chamberlain belt drive as opposed to a chain? Belt must have been twisted when installed. The cogs on the belt must all be facing the operator track, idler pulley in front, and the drive pulley. Always unplug the power before investigating. Looking out of the garage, the belt on the left sometimes twists a little from the trolley(where the red cord is) back to the motor. Just twist it back. Do not adjust tension on the belt without calling the factory, as it is preset. They WILL know if you adjust it and void your warranty. The belt itself might be bad(rare) but it does have a long warranty. Chamberlains' phone is 1-800-528-6563. One last thing. With door closed disconnect door from opener and make sure door is easy to lift up and close.

try greasing the track or the wheels. I that doesn't work then your door might be to heavy in that case get a chain drive opener
