I assume it is cheaper to start a tv channel on cable then using antennas all over the nation but I know that NBC, CBS, FOX and ABC have higher ratings because they're available to more people for free with a antenna.

Is there a book about 1) Starting a tv network like NBC, etc. and 2) Starting a tv network on cable like comedy central, etc.

There is only so much room for television stations over the air. The government has been issuing licenses since the 1940's, and most of the slots on Ch. 2-13 are filled up. That means most of the space for traditional networks is just about filled. Therefore, it would be extremely hard for a traditional network to get started. You'd have to jump on UHF channels, which traditionally haven't done as well in big cities. It's also extremely expensive to come up with enough programming to fill a network's needs -- the CW and WB had to merge and the outlet is trying to attract viewers for its shows.

Having one channel with satellite distribution has all sorts of economic advantages. Any cable outfit or satellite owner can pick it up, so start-up costs are relatively low. Of course, it's tough to get on all of the systems, but that's a programming matter.
