Tom Marvolo Riddle.
Marvolo was his mother's father's name. And then his muggle dad was Tom Riddle

Near the end of the Chamber of Secrets movie, there's a really neat little scene where diary/Horcrux Tom writes his name in the air in firey glowing characters and rearranges them to spell "I am Lord Voldemort."
