Everyone who reaches age 65 is eligible for Medicare Part A, which covers most the costs of hospitalization and there is no premium to be paid by the patient. They are also eligible to enroll in Medicare Part B which pays most of the costs of doctors' fees, lab test and the like. There is a monthly premium for that and it is deducted from the monthly pension check. If one doesn't receive a Social Security pension, then the premium can be paid every three months. They recently introduced Medicare Part D to cover prescription drug costs, with small co-pays by the patient.
I have Part A and B, along with Tricare For Life which is available to military retirees and their spouses. Because of that, all of my care under Part A and B is provided without any expense to me. In addition, Tricare For Life covers my prescription drug costs with a co-pay of $3 for generic drugs and $9 for brand name drugs for a three month supply.
Other non-military elderly can purchase Medical Supplemental policies to pick up the costs not paid by Medicare.

Medicare A for hospitals is free. Medicare B for doctors and Medicare D for drugs is available at a cost taken from the social security check. If they are low income they will qualify for Medicaid which is free and will cover the cost of B and D plus will cover all the copays and deductions.

It could be much better. Too many elderly going without the drugs they need because they have to choose between meds and food. Too sad and tragic. See it a lot as a home health worker. They have nothing.
