The history of the Elder Wand as seen in DH, is splattered with wizards gaining the wand by killing the owner. But as Ollivander tells Harry and Ron in the Shell Cottage, wand ownership is more subtle then that.

That is why Draco's wand works for Harry and Pettigrew's works for Ron: they physically disarmed the previous owners and gained the wands' allegiance. So Dumbledore, by defeating Grindelwald became it's master.

Voldemort's mistake was that he thought that Snape by killing Dumbledore has "won" the Wand. He did not know that Draco disarmed Dumbledore first and was the master of that wand until Harry overpowered Draco--as Harry explained to Voldemort.

As for the Marauder's Map, that question is not answered. I'd judge that it is a combination of the twins being very clever and the possibility of some sort of clue on the Map we don't know about.

Lupin was killed by Dolohov, Tonks was killed by Bellatrix. We know this because JKR tell us that in a online chat about ten days after the book came out. As far as I can tell, JKR hasn't said who cast the spell that killed Fred and caused so much damage to the castle. At least it is not in the book. nor in the chats by JKR I've seen.

1. Well, they're very intelligent, especially when it comes to mischief making. They must've spent hours to figure out the map and make it work.
2. One does not need to kill to own the wand - Draco's ownership of Dumbledore's wand through disarming proves that
3. These are not detailed - as that wasn't really crucial to the plot. I think Fred was just killed by the blast in the wall - no idea who fired that. I think JKR mentioned in an interview that Bellatrix killed Tonks(she'd been trying from the start), and Dolohov killed Lupin

We'll know better when she comes out with the encyclopedia.

answer number one:
MA: How did they figure out how to work the map?

JKR: Don't you — well. This is how I explained it to myself at the time, and this does sound glib. Don't you think it would be quite a Fred and Georgeish thing to say in jest, and then see this thing transform?

MA: Yeah.

JKR: Can't you just see them?

ES: But the exact word combination? Is that just a lot of luck, or Felix Felicis —

JKR: Or, the map helped.

MA: Yep, yeah. You can see them sort of answering and joking with each other —

JKR: And the map flickering into life here and there when they got closer and closer, and finally they hit upon the exact right word combination and it just erupts.

2. The same way Malfoy defeated Dumbledore, he took away his wand and made him defenseless.

3. Tonks by Bellatrix
Lupin by Doholov
Fred---not canon, assumed Rookwood.
