The first 3 speeds of the fan do not work properly. It must be on 4th speed to get any decent air flow.

This would often be diagnosed as bad resistors on the blower motor. It's more likely the switch (on the dash) is malfunctioning. Small particles of worn off metal and dust gather inside the controller over the years and can often cause the first few selections to do nothing.

You can test this by putting the air flow setting to 3, removing the knob (should pull right off) and tapping the stud that sticks out lightly with a small hammer. If the fan starts up you've got a bad/dirty switch. This can often be repaired simply by blowing it out with compressed air, if the contacts inside are badly worn the unit will have to be replaced.

If the above test does not cause the fan to come on you'll need to revert back to the "bad resistors" on your blower motor. The fix here would be replacing the resistor pack or the entire blower motor assembly.

That means the fan's resistor pack is burned out and must be replaced under the dash near the fan motor.
