When "dad" is a direct substitute for his name, capitalize, just like you would a name.

I told Dad we'd be late. Hey, Dad's on the phone. Who's seen Dad's glasses?

I told Tom we'd be late. Hey, Tom's on the phone. Who's seen Tom's glasses?

When it's a referent to the man's relationship to you (or someone else), don't capitalize.

I told my dad we'd be late. Hey, your dad's on the phone. Who's seen his dad's glasses?

I told my friend we'd be late. Hey, your secret admirer's on the phone. Who's seen his boss' glasses?

When you speak about a parent, "dad" and "mom" aren't capitalized. When you speak to them and use them like a proper noun, they are capitalized. For example, the beginning of a letter to your parents, or when the letter is closed.

No, unless the "my" is the first word of the sentence.

As for just dad. You would NOT capitalize it.

My dad is a good man.

If you are writing it about any dad, its not capitalized but if its like my dad, robert then its capitalized

nope, just "my dad". if you were going to write Dad, then capitalize.
