Just play through the normal missions for a while. When you get to the Kaineng Center in Factions, or receive the "Consulate Docks" mission in Nightfall you will be able to find someone who will give you a quest that allows you to transfer.

I don't have Factions, but the Prophecies to Nightfall quest is a toughie, I'd recommend either some powerful friends or being level 20 with some good tactics. (Those friends have to have both games involved though.)

Either play the storyline or get someone to ferry you from Kamadan to the Consulate Docks. Once there you can again get a ferry to Kaineng Center or if your lvl 17 or have the rank of Sunspear Captain speak to Mhenlo. If your not in a hurry to get there, and (being a casual player) whenever the next time I play is I will take your there for free.
i just remembered you have to get the quest "Plague in Cantha" from a npc named something Shento by the docks in Kamadan NOT the consulate. then hell redirect you to Mhenlo.

Just progress thru the Nightfall storyline until you reach the Consulate Docks mission. At that point you'll be presented with a quest in Kamadan that will allow you to travel to Kaineng City. (And another quest to take you to Lion's Arch if you own Prophecies).

I don't remember the name of the quest, but you'll get a nice big pop up box each time you enter Kamadan if it's available to your character.
