The Grand Jury: A jury of 12 to 23 persons convened in private session to evaluate accusations against persons charged with crime and to determine whether the evidence warrants a bill of indictment.
A subpoena is a command to appear at a certain time and place to give testimony upon a certain matter. A subpoena is used to compel the testimony of witnesses in a trial or other adversarial proceeding. Subpoenas are issued by the clerk of the court in the name of the judge presiding over the case in which the witness is to testify. Typically subpoenas are issued "in blank" and it is the responsibility of the lawyer representing the plaintiff or defendant on whose behalf the testimony is to be given to serve the subpoena on the witness.
The subpoena will usually be on the letterhead of the court where the case is lodged, naming the parties to the case, and being addressed by name to the person whose testimony is being sought. It will contain the language "You are hereby commanded to report in person to the clerk of this court" or similar, describing the specific location and scheduled date and time of the appearance. Some issuing jurisdictions include an admonishment advising the subject of the criminal penalty for failure to comply with a subpoena, and reminding him or her not to leave the court facilities until excused by a competent authority.

Sub poena = under penalty. A subpoena is an order to appear, under penalty if you don't ("and herein fail not, at your peril").
The grand jury is used in federal courts, and some state courts and other countries, as an initial threshold to determine if charges are warranted -- in other words, to be sure the state has a case before prosecution begins.
You are being ordered to appear personally before the grand jury, to give testimony as a witness -- and it's more than just a polite request, it is a court order to appear.

Wow, I love the language!
It means that you MUST appear before the grand jury. No excuses will be tolerated. You MUST testify on behalf of the state (the prosecutor), you must be sworn in and tell the truth. And you can't leave until the judge tells you you may leave.
'. . . and herein fail not at your peril' sounds great! It's almost pirate language! It really means '. . if you know what's good for you.' 8^)

Basically all you need to know is that you have to show up on October 18th to testify and you must answer any and all questions they ask you. You have to keep going to answer these questions until they say you are finished and dismiss you. If you fail to do so you shall be in peril of going to jail.

seems fairly simple... you have to go to the DA office in Calcasieu on the 18th of october... where you will be testifying on behalf of the state... and you will have to be available as long as the court decides necessary

Be there rain or shine without any excuses, answer all question you are asked truthfully and/or to the best of your knowledge. If you are found to have lied knowingly, you will be put in jail (under contempt, just meaning that you did whatever on purpose towards the court).

You are a States Witness if you fail to show up they will arrest you. They are preparing to indict someone and think "KNOW" you have some information that will be useful in their case.
Give a copy of this to your employeer so as not to get fired.
