I find that usually the best thing to do is just upfront, honest and straight to the point with people.
I know this because I wasted a lot of time and opportunities dithering around before I relaised this.

It seems to me that the direct approach is probably the way to go in this situation.
If it were me I'd call and say what's on my mind.
Just say something like " I've missed you and was thinking that I'd like to work things out..."
