all or none responses is when the neuron reaches -50 mv and decides to fire completely or not fire at all, and a refractory period is when the neuron has just fired and needs to collect ions to lower its charge back down so it can fire again.

Graded responses are on a scale, like 1 being against something, 3 being neutral, and 5 being for something. All or nothing is a Yes or No answer. The same principle applies if you're asking about biological reactions. A refractory period is the amount of time an organ or cell is incapable of repeating a certain action.
