your heatsink may be underperforming or the control module on the fan is giving out

first of all your fan is supposed to move fast. What you hear is probably the power supply fan, sometimes the bearings start to wear out or get dust in them and start making noise which will sometimes stop after being on for awhile. What this usually means is you need a new fan or power supply.

The bearings are worn out. Fans are relatively inexpensive, so you should probably just get a new one before it fails and leaves your system vulnerable to overheating.

It is probably going south. Time for a replacement.

You can try cleaning it with a brush (with the system off, of course).

The bushing/bearing could be wearing off, so you may want to consider changing it.
