It's a rare occasion for that to happen. It's not anything that even happens once a year, but maybe once every 5 to 10 years. I'm not proud of it, but historically, what I do is react with violence.
However, the last time somebody got on my absolute last nerve was by maliciously spitting a mouthful of chewed spaghetti in my face.
Usually I walk away before somebody gets to my absolute last nerve. I may even have matured enough to avoid letting that happen ever again.

Get away from them for a while. Take a break. Too much time around someone can cause tension. Then, once relaxed, maybe confront them about what it is that they do that gets on your "last nerve."
