I believe you won't find yourself alone in being confused by Premonition. If you read reviews, you'll find that confusion is the general opinion of most reviewers and movie-goers alike. I felt a bit disjointed at the end as well.

This is my take:

The movie is a paradox. In her attempt to understand her premonitions (distorted time-travel) and prevent the accident that takes her husband's life, her actions actually contribute to the cause of the accident. (Think 12 Monkeys and Deja Vu)

Her pregnancy is the result of her trying to mend the relationship with her husband in order to prevent his leaving/death.

It's an everyday problem. Some people question if I did this would it have changed anything. If you see a show called Journeyman he tries to change people getting killed especially one woman from the earthquake during the World Series. Problem is he doesn't realize as she doesn't by this happening it helped others in an off-beaten way. So by her trying to mend the relationship in order to save him and their marriage she one becomes pregnant and actually sets in motion what was already set to happen. But by her being given this opportunity she fell back in love with her husband and their child will grow up surrounded by love. If her premonitions would not have happened: No baby, pissed at him for going to be with another woman and possibily ruining her childrens respect for him, messing her childrens future relationships up due to her anger and pessimism. In other words no she couldn't stop the accident but she could change the eventual outcome

The movie explains no matter how hard we try we can't change our destiny. she knew her husband is going to die but she couldn't stop it.
