In the first place, the girl is not human, so it could be ANYTHING!
Secondly, in Japanese culture, a piece of paper, held in a girl's mouth means something quite nasty, and, considering, this anime character appears to be wearing a nightgown, she may be holding a rolled up piece of paper in her lips.

I don't really know for sure....but it looks like a tiny whistle or a toy flute...or it could be something else entirely different.
