No one knows but if providers continue to be paid fees for services they will bankrupt the nation. The more services they provide, whether those services are needed or not, the more money they make.
How to paid them? Capitation or a salary.

The employer does not have to be the federal government.

Well, the last time anybody pushed for total coverage for everyone, the insurance companies were on board, most of the large companies were on board, but the health care industry went into tantrums, and CEOs of those companies started calling the CEOs of other companies that the were partners with or where they sat on the board, and before you knew it, it was dust.

So I guess it comes down to when all of those CEOs stand up to the health care industry CEOs.

Any of the Democratic candidates will push through such a plan.They have a plan or two in the works. The Republicans don't have a health care plan except "don't get sick".
