In Connecticut you need to go to the Probate Court in the county you now reside in. You will be given a very simple form to fill out as to why you want to change your name; swear that you are not a convict or are changing it to get a new S.S. card or new identity or anything like that. Bring in your birth certificate so the Judge knows that you have been known by that name before. You will pay a nominal fee ($200 or less), the Judge will review it, and within a couple of weeks you should have your new (old) name back. Bada bing bada boom! Done. You cannot do it on line. A Judge must order it. It's as easy as baking a pie - no, easier. LOL It's certainly not as tough as getting divorced, either.

The easiest way would be to change it during the divorce. Since that has come and gone you will need to go to court to petition for a name change. You can not do it on-line, however you may find the forms there. Search on name change and your state.
