Well...people try to get out of crimes by claiming they are insane. And frankly, if you are dumb enough to commit a crime in the first place, then you have to be insane!
So therefore most criminals are insane and therefore it becomes a legal issue. Although they should not get any slack just becase they are insane...they are a hazard to society.

It is both -- it's a medical issue, because it involves mental health.
It's a legal issue, sometimes -- because one of the common elements (requirements) for many crimes is a particular mental state (knowledge) or intent -- and because certain mental defects can make it impossible for a person to be able to form the intent (or have the knowledge) required to be guilty of the crime.
