From my imperfect memory:
1. From Early Paleozoic to Middle Cenozoic time.
2. Not pools, but sandstone reservoirs. Also within fractured limestone reservoirs.
3. Water, natural gas, hydrogen sulfide, parafins, tars, etc.
4. By structural traps (faults and folds) and confining layers (shales, gypsum layers, etc.). Salt domes provide structural traps and confining environments.
5. It takes far longer to produce petroleum than our current usage is depleting it.
