What sort of damage to the shoe would I be at risk to?

Depending on the size of the accent and how close it is to the point where the shoe hits the ground on the heel, there are many different dangers. In my opinion I don't think it'd be wise to buy them, because of the chances that the piece could scuff, or break, or even fall off.

Plus, there are so many different alternatives when coming to pumps. But if you have your heart set on them, see if there's anyway to seal the gold in and make it less vulnerable.
