I was going to say dead or in jail, but peacedevi makes an excellent point, even if I don't like psychoanalyzing someone without their consent. Perhaps if he had grown up in a more psychologically healthy family without all the connections his father and grandfather have/had, he would not have become an alcoholic, might not have used the drugs it's alleged he used, and might have served honorably in Vietnam. He certainly wouldn't be president, I don't think, but had he grown up in a supportive loving family, he might be better off.

My answer is based upon my reading of "Bush On the Couch" where a psychotherapist analyzes Bush based on his public appearances and history.
Since much of Bush's psychological issues (excepting the severe ADD) arose from his family, namely his mother's cold fury, his father's absence, and both parents' complete inability to openly and healthfully deal with the grief from the death of his elder sibling from cancer, he might very well be a completely different individual.
For my speculation, I am going to assume that he was born to a different and much more psychologically healthy family.
I speculate that his ADD would have been openly acknowledged and treated, thus improving the likelihood that he might develop, if not an all out love of reading, at least the ability to read enough to fulfill whatever function in life he would be fulfilling. And thus, his intelligence would be increased, his speaking ability would be increased, and though he'd never be an Einstein, with a healthy and loving family, I think that he very well could have risen to a middle management position in a business, had a comfy (if not too intellectually demanding) deskjob, or perhaps, his parents would have supported him in being a healthy and happy ice-cream truck driver.
It is difficult to say precisely because so many of his difficulties lie in the specific family that he was born into.
EDIT: I forgot to mention that with appropriate treatment for his ADD, he would have had a much greater chance of NOT needing to self-medicate with alcohol.

He's really not a smart man, therefore lacking his daddy's money, buddies and connections he'd probably be a big-time alcoholic still. I can definitely picture him as a used car salesman or as a police officer, especially since the town I am from is known for having super-dickish cops

You people are idiots! What he did in his past are the same stupid things most young people do when they are growing up. Oh but I forgot, most people posting the answers here still haven't grown up. The guy did pretty well in school without his fathers help. He might have not been at the top but he did make it through, something that many ANSWERERS can't say for themselves. Final conclusion: He would have been greatly successful at something and would have had a good life.
