True you do need a Passport and visa, (common since) but the problem is getting the visa.

If you are wanting to go as a tourist you will need all kinds of papers,
bank accounts,
property ownership,
place of employment,
proof that there is a 98% Chance you will return to the Philippines and not decide to try and stay in the U.S.

You should probably call the U.S. Embassy in Manila and they could give you more information... (02) 528-6300

All that is needed is the passport and the visa. To secure your visa, there are different requirements depending on your own status. Best to discuss with your travel agent.

As a warning: Producing all of those paper requirements will not, can not guarantee that you will get a visa. The consuls are experts in determining your intentions, made worse by the Filipino illegals in the USA. Unfortunately, if the consul misreads your intention, their decision stays.

First, you need a valid passport (not expiring within the next 6 months). Then a filled-up application form (you can download it via the internet). Pay the $100 visa application fee at BPI, fill-up the supplementary form that you will get from BPI. Make an appointment for your interview in the US Embassy. For supporting documents, just go to the website of US Embassy Manila (google it). Documents needed depends on the purpose of your trip so i can't give you specifics.
