I recommend Ghost Story by Peter Straub. I actually didn't read the book, but watched the movie which was really scary. Knowing that books are almost always better - and, in this case, more frightening - I couldn't bring myself to read the book, too. Here are a couple of reviews at Amazon:
"Stephen King
The terror just mounts and mounts.
Chicago Sun-Times
The scariest book I've ever read....It crawls under your skin and into your dreams."
For something a bit less intense and scary, but scary enough for me, read House by Frank Peretti and Ted Dekker or The Oath by Frank Peretti. Frank Peretti and Ted Dekker each have a bunch of other books that fall into the creepy category, too.
For a true story that was really creepy, read The Amityville Horror by Jay Anson (the movie didn't do it justice in my opinion).
Other scary stories would include some books by Stephen King like Pet Semetary; The Shining; Salem's Lot and more.
Some of Edgar Allan Poe's stories are scary and/or creepy, too. For instance, you may want to read The Fall of the House of Usher.

Oh, so many! Books by Stephen King. I also liked Saving Helen by Mary Downing Hahn. A ghost story that's meant for younger-than-you readers, but still really, really good! Definitely haunting. Time For Andrew or something like that, was also very good and scary, but not horrifying. The spooky type of scary.
As for movies, try Hide and Seek with Robert Di'Nero and Dakota Fanning or 1408 with a bunch of famous actors. In Hide and Seek, someone is killing the people around the two main characters and Dakota's character, Emily, blames it on someone named Charlie. 1408 (or some number of that kind) is about an author who rents hotel rooms that are declared haunted and tries out that ^ room number and ends up finding out that it really is haunted.

ck1 took the words out of my mouth...I'm reading The Amityville Horror right now and it's fantastic, and pretty creepy. Someone asked a question on this recently (link below) so here's a few more options. I like Stephen King, The Children of the Corn and The Boogeyman.

"Tommy Knockers" by Stephen King is great. Also "It" is very good. Is you want something incredibly sick and twisted try "The Beast Within" by Edward Levi.
