I have watched the movie i just did not see any conflict.

External : Is it worth the money and energy to experiment on a patient that is considered a lost cause.

Internal: If you find out that the help you can do will not last, and the patient will be right back where they were, is it worth getting everyone's hopes up?

In the movie this doctor tries ANYTHING that might work.. costing a lot of money to the facility, which is short on it as it is.. and he manages to help, but when it turns out that it was only for a short time.. was the money worth it?.. Giving him one last chance to say goodbye, while getting his (and his families) hopes up. The movie does not tell you which belief is correct.. it's up to the watcher..

Are you referring to the movie with Robin Williams & Robert De Niro? If so, my perceptions are as follows.
EXTERNAL: Using unconventinal or medically approved expierements on unknowing patients. Not having his superior in full compliance by lacking support of the Dr.'s plan of action, or patient's immediate care.
INTERNAL: Doctor allowed himself to become emotionally attached to his patient's. ( as well as their families, especially DeNiro's mother. ) As the Dr.'s patient's heath began to decline, so did the confidiance of their Dr.'s. which he recovered from. Knowing, in his heart, that by taking the risks, for a brief moment in time ... his patien's were freed of their bodies that held them hostage for so long.
