I know I am not suppose to use the client's name.
Do I write it in paragraph form or in list style. How do most psychologist write a client's narrative??
Any websites with examples???? Any help would be GREATLY appreciated !!!

A narrative is usually a paragraph that tells the age, sex, marital status, summary of history, particularly as it refers to the diagnosis or diagnoses, the tests performed and conclusions and your possible conclusion.

For example,

J. R. was single man, twenty-three yr. of age, complaining of hearing voices and requiring supervision to ensure safety. He said that he had been hearing voices since he was 13 yr. of age and he attended special education classes throughout his academic career. A CAT scan revealed large vacuous areas in the frontal and temporal lobes from a traumatic brain injury. These symptoms have been treated with anti-psychotic medication, namely Risperdal and Seroquel with limited positive effect. The conclusion is that the person has neurological dysfunction resulting in psychotic symptoms with and there is little hope of a positive prognosis. The person may continue to require supervision.
