Yes that happens. My mouth is always running and my brain is two steps behind.
I don't get in trouble too much because I have a smile on my face all the time and people don't know if I am kidding or not.

Oh lordy yes. Far too many times in my opinion..despite my best efforts to work at keeping my mouth shut until I've had a chance to think about it.
Sigh.
usually it's a joke gone bad, or taken the wrong way, but not always.

yep...i,m a Brit now living in America..only been here three months...but when i first arrived i said to someone "thanks c*ck"..where i come from(NR Manchester)that's an everyday saying..it means thank you....but apparently..over here its an insult..ooops
