In Silent Hill, she starts bleeding from the forehead and stumbles to Harry, who exits the room in a panic. Afterwards she just disappears. If you complete de Kaufmann sidequest (save him at Annie's Bar), Lisa reappears (bloodied and mute) at Nowhere, after Harry defeats the final boss, and she drags Kaufmann into a void. Lisa appears in archive footage in Silent Hill 3, and again in Silent Hill Origins (which takes place 7 years before the first game, so of course she is alive and well then, although an implied drug addict). The implication is that Lisa is dead well before the first game (sometime between Origins and SH1), but that she is, for some reason, unaware of it. She is dead, a monster like the other Puppet Nurses in Alchemilla. She realizes this during SH1 and "goes dead". So she's been dead all along.
