It depends on the person who is saying it. If it's someone you now then whatever however, if it someone douche bag then don't say anything back to him our her unless, you like him our her; just walk away and if they're still persistent you tell someone you're being harassed. Our if there is nobody around then you grab your cell phone and dial 911! I'm sure you have a cell phone right, um like what planet are you living on like for real.

It depends. How would I be feeling that particular question?
I would say what I could best put together. Like, "It's been slow", "P*ssed off", "Pretty dandy, friend.", etc. That's usually followed by a "Thank you" and reciprocation of the same question, or an exit.
For example,
"It's been slow. I appreciate your interest, friend. And how have you been?"
Well, you know what I mean. :)
Just sort of whatever fits for the moment. :)

Generally it's just a greeting, know one expects to hear how you're really doing. If the person asking is a close friend they may really mean it but if it's only an acquaintance it's basically just an expression of politeness.
My answer to the question of 'how are you' is usually 'great, how are you?'

I usually say, "Fine, thank you how are you? But, if I'm not chipper enough to stomach this I just say, "doin" or "I'm here" and quickly change the subject.
