There is no real accurate method of measuring pain because every-ones tolerance to pain is different.
The closest you can get is, figure out what the most intense pain you have ever felt and use that as a ruler making it a 10 with 0 being no pain at all.
Then within that scale measure the level of what your currently feeling.
To some a headache is a 10 to others it is a 1. It all depends on your tolerance level for pain.

I also believe that it is different for everyone, some people have no idea what pain is, or have been around anyone who has ever suffered with it.
I believe that mild is when it is tolerable to where you can participate in life and enjoy family and friends, moderate is when you have lack of sleep due to the pain and need to go to sleep to escape from it though it may be a very short time you are able to sleep, extreme is where you can see it in the face of the person, the face is tense and tight and they are to the point of vomiting or past that point, nothing makes them comfortable.

I have chronic pain so it's 1-10 when I describe it to the doctor. At home it's measured by whether it's heaven or hell. Just ask family members they can verify it lol.

actual discomfort is in truth an electric powered reaction contained in the mind, top? Emotional discomfort is perchance similar -- chemical substances interacting at the same time. it quite is the clinical reaction... notwithstanding the logician Miguel de Unamuno, author of the Tragic experience of existence, tells us that people are "flesh and bone" and born to large suffering and shortage of life. He tells us existence isn't completely measurable, definable, comprehensible. a tactics from being a foul component, though, Unamuno shows that it truly is that this undesirable, emotional, mystical idea of existence which has the most meaning. So... do we degree discomfort? not contained in the truest experience, i imagine. Measurements fall short after we observe them to the human journey. existence is a lot previous what we are able to draw close. existence is a risky, mystical, emotional journey. technology has tamed this reality somewhat... yet perchance not as a lot as lets favor to imagine.
