Do you have to go to the place where you were originally registered as a baby? And what ID do you need to take with you? Do you know how much a new one costs?

This happend to us when we moved, you have to contact the Births & Deaths Registry office where you registered make a appointment & go with your partner ( if not married & your using his name for the baby). They will issue another one but I think it cost about £20.00/£30.00, but that was a few years ago, now I keep everything together in a safe place!

I just got a new one last year. Just take ID and SS# to county registrar where you were born. Think it cost about $20.

f you live within the area you were born a replacement costs £7.50.
If you were born in another area then check your local council offices. They will be able to give you the information you need.

call the county court house and ask to speak to the department who handles the birth records in the state where you live
( various states have different departments ) where I live it is the health department. talk to one of the clerks and they can inform you of the procedure and cost.

the hospital has copies, or you can go to the courthouse in your town and get a copy....as long as you have another form of ID I think, or your parent is there, if you are young
