What has always helped me is to make study notes as if you were going to be able to use them on the test, even if the exam is not open notes. For me it just helps to organize the information and write it down in order to visualize and understand it better. (2nd to last semester of Graduate School)

.. Organize your notes. Use them with the text books if ou have to clarify some things.
.. Put aside a study time each day, and use it only for study. Be disciplined about this.
.. Make sure you haven't fallen behind in any assigned readings. Your instructors have the right to test you on all the material, not just the stuff you heard in lectures.
.. Make sure to get a good night's sleep before each exam. If you're not rested up you won't do well on the exams.
.. Do not try to cram everything in at the last minute. Cramming doesn't work as well as people think it does.
.. If any of the exams are "open book" have your material organized and at the ready.
