A size of 40 should mean Large but some brands have 40 for Medium size too.

I agree with the second answer, which says "based on chest size."
In a polo shirt I like a large, in a dress shirt I prefer 17.5 inch neck with 32/33 sleeves, and my sports coats or jackets are a size 40.
