The problem with those games is that whoever is involved must set some rules and a very important one that is when to call it off. Each of you must pre-set a phrase that calls off the game. If you are into domination, then the word “stop” won’t make it, but something different like “airplane”, or “ocean”, or anything that has nothing to do with the game and that signals that the game must stop for whatever reason.
But it is also important in this case to set some limits or boundaries. Like not to make fun of each other. You now have a scar on you and your sexual performance is going to be affected by it. You have no other option but to either accept what happened and move on, or talk to your wife and let her clarify what happened.

When I was in martial arts, and I couldn't perform a technique properly, I would argue that it's too hard. My sensei would simply reply with, " Well, get good at it. Don't whine and have a pity party." So from then on I decided that if something's hard, I will make it easier. So you do the same. Too many people are taking marriage for granted and divorcing like their dating. This is a partnership. Maybe you two forgot about what it was like to be best friends again. Find that. so you can't handle it. then stop doing it.
or you can draw lines that shouldn't be crossed. in "rape" senarios (yes, some sickos do do this) there is a code word, like "red" that calls stop to all activity. usually used when one person is being hurt or gets freaked out.
establish a code word and some guildlines. make sure she has a list too so you don't feel uncomfortable

Yeah, let her know that's a subject you are ultra sensitive about, and thus it shouldn't be made light of even in sex play. Might as well consider anything else she might say that could hurt your feelings and make those topics "off limits" also.
OR find a different role play game because humiliation is often part of what subs go through, and you don't appear to be cut out for it.

You should tell her there needs to be some rules for this role playing game. Somethings should be off limits. Anything that is said that makes you feel bad about yourself is off limits. Yeap you are going to have to ask her if she really feels this way about your privates or it's going to drive you crazy.

I think you should talk to her... if shes the dominating partner, she could have been saying things like that to make you feel more submissive, but she may not really mean ANY of it... but at least talk to her about it... If she does really mean what she said, it obviously doesnt matter much to her if you still have a stimulating sex-life.

They style of role playing is meant to break you down completely. To bring all your emotions to the surface. Its a personal journey, to make you stronger and shows you what is needed to improve you. What hurts your feelings is your own way you think about you. If you need to feel better about something certain, find a way to make that thing better or in your case...bigger. If you no longer feel weak about that one item, you mentally will be more confident in yourself.
