It's not a question of etiquette... It is a question of practicality... The quilt is more difficult to clean than the sheet; therefore, the top sheet keeps the quilt more fresh for a longer period of time...

No, you don't need a sheet as well as a quilt. If you use blankets instead, then you need a top sheet too.

I usually have a sheet somewhere around the bed, it comes in handy for when things get really hot, plus i quite like to be wrapped in it and rubbed xx ; - ) xx

I just have a duvet and when it gets cold i use a blanket too. I don't think there is any specific eitquette, just whatever you find comfy.
